feat(ui): mcp_tools/OpenAPIFormSection Input to shadcn

Browse source 
- antd Input \u2192 shadcn Input. antd Form/Form.Item + FormInstance
  preserved (parent antd Form context + setFieldsValue calls in
  handlePresetSelect).
