Add integration test for _has_applicable_rate_limits coverage

Add comprehensive integration test `test_has_applicable_rate_limits_integration_with_async_pre_call_hook` that exercises _has_applicable_rate_limits through the realistic async_pre_call_hook code path.

This test verifies that the method works correctly in both scenarios:
1. No limits configured - file retrieval is skipped
2. Various limit types (RPM, TPM, user, team, organization) - file retrieval is attempted

This ensures full code coverage of the _has_applicable_rate_limits method which checks for 6 different rate limit categories (12 conditional checks total).

Test cases cover:
- No limits (returns False path)
- API key RPM/TPM limits
- User-level limits
- Team-level limits
- Organization-level limits
