fix(guardrails)!: read Gemini contents in the Lakera v2 guardrail

Lakera v2 built its inspection messages from messages, Responses input and
instructions only, so a Gemini generateContent body (systemInstruction and
contents) on /gemini, /vertex_ai or the native generateContent route sent
no text and the request reached the provider unchecked. It now reads the
system instruction and each turn's text parts, with model read as
assistant. Gemini contents have no write-back path, so mask and
inject_system_message block on that shape, and monitor forwards it
unmasked with the reason logged.
