fix: handle tool_use/tool_result blocks in r1-format for DeepSeek Reasoner

This fixes issue #10101 where DeepSeek Reasoner (and other models using
R1 format) would get stuck in a loop repeatedly calling update_todo_list.
The root cause was that convertToR1Format did not handle tool_use and
tool_result blocks. When native tools were used:
- tool_use blocks in assistant messages were being ignored
- tool_result blocks in user messages were being ignored
This caused the model to not see its previous tool calls and their
results, leading it to keep calling the same tool repeatedly.
The fix adds proper handling for:
- tool_use blocks: converted to OpenAI tool_calls format
- tool_result blocks: converted to role: "tool" messages
This ensures the model receives proper feedback about its tool calls
and can proceed with the task instead of looping.

fix: extract raw error message from OpenRouter metadata ( #10039 )

OpenRouter wraps upstream provider errors in a generic message but includes
the actual error in metadata.raw. This change:
- Adds OpenRouterErrorResponse interface for proper typing
- Creates handleStreamingError() helper for DRY error handling
- Extracts metadata.raw for actionable error messages in PostHog
- Includes nested error structure so getErrorMessage() can extract raw message
Before: PostHog receives '400 Provider returned error' (generic)
After: PostHog receives 'Model xyz not found' (actionable)
This enables proper error tracking and debugging via PostHog telemetry.
