Replace getKeepMessagesWithToolBlocks with simpler getKeepMessagePairs that
extends the keep range when the first kept message is a user message. This
naturally keeps assistant tool_use and user tool_result pairs together without
requiring complex tool-specific logic.
The previous implementation could split tool_use/tool_result pairs when
summarizing, causing 'tool result's tool id not found' errors when the API
received a tool_result without its matching tool_use.
Changes:
- Renamed getKeepMessagesWithToolBlocks to getKeepMessagePairs
- Simplified logic: extend keep range to include assistant message before user
- Removed useNativeTools parameter (no longer needed)
- Updated tests for new behavior

OpenRouter wraps upstream provider errors in a generic message but includes
the actual error in metadata.raw. This change:
- Adds OpenRouterErrorResponse interface for proper typing
- Creates handleStreamingError() helper for DRY error handling
- Extracts metadata.raw for actionable error messages in PostHog
- Includes nested error structure so getErrorMessage() can extract raw message
Before: PostHog receives '400 Provider returned error' (generic)
After: PostHog receives 'Model xyz not found' (actionable)
This enables proper error tracking and debugging via PostHog telemetry.
