docs: add documentation freshness guide with steward workflow (closes #74)
Inspired by Monika Voutov's BoardKit Orchestrator — 'stale docs = hallucinating AI'
- Doc update checklist for every task completion
- Freshness headers format: date | version | updater
- Three-phase automation plan: manual → hook-based → AI doc steward
- Repo rules (CLAUDE.md equivalent) for agents
- Trigger matrix: when to update what
