Docs: final URLs for the v5 banner and the changelog link; permanent /connectors redirect

From the Search board inbox:
- The v3/v4 banner linked /migration/api-v5, which 301s to /docs/migration/api-v5. It sat on every docs page, so 200
  links went through a redirect.
- The navbar's changelog link lacked the trailing slash (a 308 from every docs page).
- /connectors answered with a temporary 307 to /connectors/overview. It's now a permanent redirect.
