feat(proxy): let a Claude Code gateway user choose the session team (#49001) 
* feat(proxy): let a Claude Code gateway user choose the session team

The gateway's browser verification page now lists the developer's teams, and the device code grant mints the session for the picked team. A team the developer is not on is refused with a 400, and a form without a team keeps the first-team default.

* fix(proxy): refuse a stale gateway team pick and require an explicit choice

* fix(gateway): list the browser-picked team first so an older replica mid-deploy issues it
