refactor(logging): build the logger registry mapping lazily instead of lazy entries 
The two dynamic rate limiter entries are real classes again. The whole mapping
is built on first read, so the proxy hooks still load only when the registry is
used, and proxy_server.py reads the dict as before

In total litellm runs 1000+ tests

[02/20/2025] Update:

To make it easier to contribute and map what behavior is tested,

we've started mapping the litellm directory in tests/unit

This folder can only run mock tests.
