fix(proxy): exercise guard wiring in CI via stub module test 
The config-building and delegation paths only ran when fastapi-guard was
installed, which litellm's CI never has (codecov patch 70%). A stub guard
module injected through sys.modules exercises the same wiring everywhere:
env parsing, zero-trust proxy default, redis-off default, exclude-path
default, and delegation through the middleware.
