Yujong Lee 204f146dc9 refactor(rust-error): keep causes as sources in llms and the gateway crates ... ErrorDetail loses its string variant, OCR provider variants become data, host faults stop counting as the caller's, a tunnelled transport error classifies the same on every stream path, and each gateway crate has one Error with one status() and extractors that reject in its envelope.
