mateo-berri dd6d904f91 fix(proxy): give spend logs and callbacks one shared session id 
When no session header or metadata.session_id arrives, pre-call now
writes one fabricated uuid to both litellm_session_id and
metadata.session_id, so SpendLogs.session_id and the Langfuse trace
sessionId match. A body-only metadata.session_id is promoted onto
litellm_session_id the same way. New opt-in general_settings flag
enforce_session_id rejects session-less requests with a 400 before any
id is fabricated.
