Shin 78983b1678 fix(streaming): wait for OpenRouter usage chunk before StopIteration 
OpenRouter sends usage data in a separate chunk after the finish reason
chunk. Previously, LiteLLM would raise StopIteration too early, causing
the usage chunk to be missed and falling back to inaccurate token
estimates from the internal counter.

This fix:
- Adds has_received_usage_chunk flag to CustomStreamWrapper
- For OpenRouter with stream_options.include_usage=True, continues
  iterating until the usage chunk arrives
- Once usage chunk received, allows StopIteration as normal

Other providers are unaffected.

[02/20/2025] Update:

To make it easier to contribute and map what behavior is tested,

we've started mapping the litellm directory in tests/test_litellm

This folder can only run mock tests.
