yucheng 1c2ebb37b4 fix(logging): scrub the request's prompt out of failure error text when message logging is redacted 
When message logging is redacted (globally, per request, or per callback), the
failure path still sent the provider's error text, which often quotes the
prompt, to every callback and to the proxy's spend log, DataDog and OTel
server span. Scrub the request's own text out of error_str, error_information
and traceback_exception instead of blanking them, so the error type, status
code and the rest of the wording stay.

Also carry a per-request redaction opt-in over to the proxy-only failure log,
whose field allowlist left it out.
