Julio Quinteros Pro d8d3375a3c Add missing migration for LiteLLM_ToolTable policy changes 
PR #22732 changed the ToolTable schema (renamed call_policy to
input_policy, added output_policy/user_agent/last_used_at columns,
updated indexes) but didn't include a migration for these changes.

Additional files for the proxy. Reduces the size of the main litellm package.

Currently, only stores the migration.sql files for litellm-proxy.

To install, run:

pip install litellm-proxy-extras

OR

pip install litellm[proxy] # installs litellm-proxy-extras and other proxy dependencies

To use the migrations, run:

litellm --use_prisma_migrate
