Usage tracking, compliance and the dashboard now treat both not_run (older spend logs) and skipped as unevaluated through a shared UNEVALUATED_GUARDRAIL_STATUSES set, so old records stop counting as passed. The skipped record is no longer written when the request carried images, since images without text were never dispatched to guardrails before this change and that gap is not a message-scoping skip

The per-guardrail status a scoped-out evaluation records is now skipped, matching the
skip_*_in_guardrail settings that cause it. Request-level rollup still maps it to not_run
so the StandardLoggingPayload status contract is unchanged

A not_run entry from the base guardrail only carries guardrail_name, while the
content filter's evaluated entry carries guardrail_id. Keyed apart, one request
listed twice in the monitor for a logging_only guardrail (Not run and Passed).
Resolve the id from a same-name sibling in the payload so the severity pick applies

One flush drains up to MAX_LOGS_PER_INTERVAL source transactions or logs, but a
transaction fans out to one LiteLLM_SpendLogToolIndex row per tool and a log
to one LiteLLM_SpendLogGuardrailIndex row per guardrail, so the index
create_many payload was unbounded. Both index writes now go through
spend_log_write_batches(SPEND_LOG_WRITE_BATCH_MAX_BYTES, SPEND_LOG_WRITE_BATCH_MAX_ROWS).
The tool index write moves out of the rollup batch_() so the split reduces
the query-engine payload; replayed index rows are no-ops under
skip_duplicates, and the daily rollup upserts stay in one transaction

Custom code guardrails could only allow(), block(reason) or modify(). This adds flag(reason, metadata={}) which lets the request or response through unchanged and records a guardrail_flagged entry carrying the guardrail name, configured mode, evaluated input_type (request or response), reason and structured metadata. The new status is threaded through the request-level guardrail_status aggregation, the Guardrails Monitor rollup (flagged_count), Request Logs (action=flagged, most severe phase wins when a guardrail runs pre and post call) and the Request Logs detail view in the dashboard, which now renders FLAGGED with warning styling instead of falling into FAILED.

A row that received both priced and unpriced increments used to collapse
to cost NULL, throwing away the priced subtotal and making every unit on
it read as untracked. The rollup now carries a second column,
untracked_units, that the aggregator increments for units with no known
price while cost keeps accruing for the rest, so cost covers exactly
units - untracked_units. Rows written before the migration keep cost
NULL and still read as untracked in full
The endpoints read untracked units off the column (or the whole row for
a legacy NULL) rather than from a NULL filter, and the policies overview
now fills totalUntrackedUsageUnits, which the previous commit missed

A counter missing from the cost map entry was priced at 0.0 per unit, so
the rollup recorded it as known-free usage. It now stamps None for that
counter and the rollup writes NULL, while the per-request guardrail_cost
that feeds spend and budgets still sums only the known prices.

The daily guardrail usage rollup stored billable units per counter but no
cost, so the usage endpoints could only report units. The Bedrock hook now
stamps guardrail_cost_by_unit next to guardrail_usage, the spend-log
aggregator sums it into a new nullable cost column on
LiteLLM_DailyGuardrailUsageUnits, and /guardrails/usage/overview and
/guardrails/usage/detail/{id} return cost, totalCost and cost_by_unit /
cost_by_team / cost_by_key alongside the existing unit breakdowns.
Cost is nullable on purpose. Rows written before this migration, and rows
whose hook had no pricing entry, read as null rather than $0, and a single
unpriced increment keeps that row's cost unknown instead of partial.
guardrail_cost and the spend/budget path are untouched.

The daily guardrail metrics and usage-unit upserts are non-idempotent
increments, but the retry loop re-sent every failed row on any exception.
An ambiguous post-send failure such as a read timeout after the write had
already committed therefore stacked a second increment and inflated the
billable unit totals served by the guardrail usage endpoints.
Retry only DB_RETRY_SAFE_ERROR_TYPES (httpx.ConnectError), the same rule
the spend writer and autorouter rollup use for increment upserts, and log
any other failure once as terminal for that row while the rest of the
batch still lands.

A transient DB error during the spend log flush dropped that batch's guardrail
metrics and usage unit rows for good. Retry only the rows that failed, up to 3
times with 1s/2s/4s backoff, mirroring the daily spend writer, and inject the
sleep so tests stay fast. Lowers the lint budgets the refactor freed up
The flush and the usage endpoints summed units with a scan per distinct key,
quadratic in rows times keys; group sorted rows instead. Skip payloads without
a request_id like the metrics path, type the flush key as a NamedTuple, and drop
the (guardrail_id, date) index that the primary key already covers
