fix(ci): revert proxy-extras to 0.4.50 and add missing env key doc

Revert litellm-proxy-extras version bump to 0.4.50 (matching PyPI) —
the publish is handled by a separate GH Actions workflow, not the CI job.
Add missing PRISMA_RECONNECT_ESCALATION_THRESHOLD to environment settings docs.
