test(cohere): use unique test module basename to avoid pytest import clash

Rename test_transformation.py to test_cohere_rerank_v2_transformation.py;
sibling provider dirs have no __init__.py, so a bare test_transformation
basename collides with other same-named modules in the same CI shard
(import file mismatch).
