fix(guardrails)!: make lakera_v2 on_flagged block reject PII-only detections, add mask (#48944)
* fix(guardrails)!: make lakera_v2 on_flagged block reject PII-only detections, add mask on_flagged block now returns 400 when Lakera flags only PII, instead of masking it and forwarding the request. on_flagged mask keeps the old behavior: mask PII-only detections and block anything else. * test(guardrails): cover lakera_v2 on_flagged on every endpoint, hook and a Lakera outage * test(guardrails): type the lakera_v2 on_flagged unit test helpers
