Subject: [PATCH] fix(xai): correct grok-4.5 cached-input pricing

cache_read_input_token_cost was 5e-07 ($0.50/1M) and
cache_read_input_token_cost_above_200k_tokens was 1e-06 ($1.00/1M).
xAI's current pricing page lists $0.30/1M and $0.60/1M respectively
for these two tiers; base input/output pricing was already correct.

Verified 2026-08-10 against:
- https://docs.x.ai/developers/pricing
- https://docs.x.ai/developers/models/grok-4.5
- https://docs.x.ai/docs/models (this entry's own `source` field)

All three currently agree on $0.30 / $0.60.
