fix: improve thinking tag filtering to prevent content loss
- Replace incomplete regex patterns that only removed opening/closing tags - Add proper regex to remove entire thinking tag blocks including content - Handle partial thinking tags during streaming - Clean up extra newlines that result from tag removal - Add comprehensive test cases covering all scenarios - Fix TypeScript error by adding default case to toolDescription function Fixes #5813
