feat: add Codex-compatible plugin
Add gitnexus-codex-plugin/ with:
- .codex-plugin/plugin.json (Codex manifest)
- .mcp.json (MCP server config)
- 7 ported skills from gitnexus-claude-plugin/
- .agents/plugins/marketplace.json (plugin registry)
Update README with Codex plugin installation instructions.
