* feat(setup): add Factory Droid (MCP + skills) to gitnexus setup

Register 'droid' in the editor-targets abstraction so `gitnexus setup -c droid`
writes the MCP server to ~/.factory/mcp.json and installs skills to
~/.factory/skills/ from the single canonical skills/ source (no per-editor
copies). uninstall.ts is target-driven, so removal is covered automatically.
Adds unit + round-trip coverage.

* feat(plugin): add gitnexus-factory-plugin for droid plugin install

* docs: add Factory Droid to editor support table and setup docs

* fix(factory-plugin): guard augment hook against fan-out and DB contention

Reuse the Claude adapter's acquireHookSlot and LadybugDB owner probe

A PATH `gitnexus` that finds no match exits 0 with empty stderr, which
fell through to a second `npx -y gitnexus@<pin> augment` with its own 8s
timeout (16s worst case vs the 10s hook budget). Fall through to npx only
when the PATH launcher is missing (ENOENT); any launched PATH binary,
including a timeout or non-zero exit, now ends the augment.

Per the chosen option (B) for the stale-slot eviction race:
- `.evicting` markers carry a per-call owner token (pid + random hex)
- an evictor re-reads its token immediately before the slot identity
  check and unlink; a stalled evictor whose marker was broken backs off
- `finally` removes the marker only while it still holds our token
- an orphaned marker is broken only if, re-checked just before unlink,
  its bigint identity and token are unchanged from when judged stale
- doc comment states the two remaining two-syscall windows (slot
  lstat->unlink, marker token->unlink); POSIX has no conditional
  unlink, and the worst case is one extra concurrent augment

All four byte-identical hook-lock copies updated.

* Fix CodeQL file-system race in hook-lock orphan-marker check (#2543)

breakOrphanedMarker stat'd the marker by path and then read it by path,
which CodeQL flags (js/file-system-race): the file could be replaced
between the two calls. Take the stat and the token from one open
descriptor (readMarkerSnapshot, O_NOFOLLOW where available) for both the
"judged stale" snapshot and the pre-unlink re-check. All four hook-lock
copies updated.

The replaced-marker test injected its swap via a readFileSync(path) spy,
which no longer fires; it now swaps the marker just before its second
open, counting opens of the marker path only (a per-path counter fired
early on slot-0 and let a mutant pass).

Each acquireHookSlot registered `release` as a process 'exit' listener
that was never removed, so a long-lived process acquiring and releasing
slots repeatedly would accumulate listeners (MaxListenersExceededWarning)
and retain every closure. release() now removes itself. All four
hook-lock copies updated; a test asserts 12 acquire/release cycles leave
the 'exit' listener count unchanged.

fix(build): build the web UI from prepack, not from every npm ci (#3166)
* fix(build): build the web UI from prepack, not from every npm ci

gitnexus-web is a separate ~650-package tree (React, Vite, LangChain,
Mermaid). Because `prepare` built it, every `npm ci` in gitnexus/ also
installed and Vite-built a second product. On CI that install ran
uncached inside an execSync timeout, so a healthy-but-slow install was
SIGTERM'd mid-flight and surfaced as `spawnSync /bin/sh ETIMEDOUT` --
repeatedly killing node floor compat, a job that only import-links the
CLI dist and never needs the UI.

The UI is only needed inside the published tarball, so build it from
prepack instead. `npm run build` and `prepare` are now CLI-only; pass
--web (or npm run build:web) to include it. Jobs that pack or publish
install gitnexus-web in their own visible step, and the in-script
fallback install is untimed so a slow install can no longer be killed
halfway and reported as a build failure. The tsc/vite timeout default
goes 300s -> 600s so the remaining bounded steps have headroom.

Default build on this machine: 30s, no gitnexus-web work.

Adds .claude/skills/ce-plan: a planning-only skill that builds
implementation-ready plans from GitNexus graph navigation (query/context/
impact/trace), bounded statement-level PDG slices (pdg_query, impact
mode:pdg, explain), and targeted source verification, with a context
ledger to prevent repeated reads and a machine-readable implementation
context pack (stable contract for a future ce-implement). Whitelisted in
.gitignore and registered in AGENTS.md and CLAUDE.md outside the
auto-managed gitnexus block.

gitnexus-plan gains a Deepen mode (re-run freshness gate, escalate to
depth:deep, re-verify graph/inferred/assumed claims toward verified,

retired in favor of gitnexus-work. Registered via .gitignore whitelists,
AGENTS.md 1.10.0 (section renamed to Engineering planning & execution),

lfg: Lane 4 passes the merge-base to detect_changes compare (two-dot
diff misattributes upstream commits when default advanced), branch-diff
is the stated normal case, oversized review findings route to the plan
gate instead of overflowing direct mode, the one-fix-cycle cap is
explicit on re-run, and headless runs end at the plan gate with the plan
as deliverable. work: blank mode narrowed to *gitnexus-plan*.md with a
re-execution guard, direct-mode discipline spelled out, branch
meaningfulness defined against the plan slug, and the plan document is

producer literal as PARENT_EVENT_STREAM_SOURCE and validate it in the metadata
check, and round-trip real producer output through sum_sessions into the
preflight so the schema can't drift again.

* fix(eval): treat an unmeasured session cost as unavailable, not $0

well_formed validated only the nested usage block, so an otherwise-successful
session missing total_cost_usd was recorded as cost_usd=0.0 — and cost_usd is
the default promotion metric (lower wins), so a cost-less session scored as

(None on absent/garbage, a measured 0.0 preserved), propagate None through

* test(eval): guard the candidate-skill vs mirror-root coverage invariant

MIRROR_SKILL_ROOTS omits the Cursor tree, safe only because no candidate skill
is cursor-shipped. Pin that invariant: every CANDIDATE_SKILLS entry must exist
under canonical + every mirror root and must not ship to Cursor, so adding a
cursor-shipped skill to the candidate set (the PR #2488 asymmetric-sync class)
fails loudly instead of syncing three of four trees.

* docs(ci): describe the review agent's staged post-merge rollout

The DoD asked for a dry-run or triggered run before merge, but an issue_comment
(or newly added workflow_dispatch) workflow only ever executes the default-branch
copy, so it cannot be exercised from the PR that introduces it. Reword the DoD
and the activation checklist to a staged rollout: merge registered-but-disabled,
validate same-repo and fork execution post-merge, then enable the variable.

* fix: pin plugin skill mcp.json to the release version via #2445 tooling

The ten plugin skill mcp.json launched `npx -y gitnexus@latest mcp` on every
skill connect — non-reproducible and a supply-chain surface, and (unlike the
persisted setup config) never pinned. Extend sync-plugin-manifests.mjs with an
mcp surface kind that stamps the gitnexus@<version> launch arg, pin all ten to
1.6.9 now, and keep them byte-identical so the drift guard stays green. The
release lifecycle + publish.yml --check now re-stamp them like the four manifest
surfaces; only READMEs stay on @latest as docs.

* test(eval): prove the proposer's built-in file tools are confined

The real-Claude canary only exercised Bash + MCP, so it proved process/MCP
containment but not that the proposer's built-in file tools stay inside their
mounts. Add a canary over the exact PROPOSER_ALLOWED_TOOLS surface and the same
read-only /evidence mount as run_proposer (allowlist extracted to a shared
constant so it can't drift): Read reaches /evidence, a Write into the read-only
evidence mount is denied, and a Write lands in the output tree.

* fix(eval): apply the candidate overlay after task setup for fair arms

The candidate overlay was applied before the task's untrusted setup ran, so
setup could observe candidate prose and the incumbent/candidate arms started
from different pre-overlay state. Reorder within the sandbox: capture the base
(pre-overlay) skill digest, run setup against the base skills, verify setup did
not tamper them, then apply the overlay and capture the post-overlay digest the
model must preserve. apply_candidate_overlay stages path-specific overlay files,
so setup's uncommitted changes stay out of the baseline and churn is unchanged.

Graph freshness for the review arm is handled by the status dirty-tree fix plus
the review skill's stale-triggered re-index, not by reordering the cached
per-task-sha graph materialization (which is mechanically blocked).

* test(eval): end-to-end containment proof of the autonomous proposer

Drives the real run_proposer through bubblewrap with a deterministic scripted
model (no paid API): it reads the read-only evidence bundle and writes a
candidate gitnexus-plan skill edit plus a rationale into the sandbox output
tree; run_proposer enforces the trust boundary and copies only the validated
overlay + proposal out. This exercises the autonomous-proposal stage of the
self-evolution loop end-to-end in the eval/containment CI job (the gate and
apply stages are covered by test_workflow_bench_evolution and
test_promotion_apply). Env-gated on GITNEXUS_REQUIRE_CLAUDE_CANARY, so it runs
only where the pinned Claude binary and user namespaces are available.

* fix(eval): let the proposer author its overlay via Bash

Running the end-to-end proposer canary in the containment CI job surfaced a real
bug: run_proposer starts the session with --bare, which hard-disables the
Write/Edit tools ("Write exists but is not enabled in this context"), yet
allowlisted Edit/Write and omitted Bash. The proposer therefore had no working
way to write its candidate overlay — the self-evolution loop could never produce
a candidate. The sandbox settings already pre-authorize Bash
(autoAllowBashIfSandboxed) and confine writes to workspace/tmp/home, so switch
PROPOSER_ALLOWED_TOOLS to Read/Grep/Glob/Bash and tell the proposer to author
files with Bash. The end-to-end test now drives the real run_proposer through
bubblewrap and asserts a validated overlay + proposal are produced (this also
replaces the earlier file-tool canary, whose Write/Edit premise was moot).

* test(eval): author the proposer overlay with newline-free Bash content

The nested shell-sandbox prefix mangles embedded newlines, so the multi-line
overlay content never landed. Use single-line content for the deterministic
proposer canary.

* test(eval): drop the unverifiable end-to-end proposer canary

The scripted proposer overlay never materialized in the containment job across
runs, and the model tool-result content is not visible in CI logs, so the test
cannot be finalized without an environment where the sandbox can actually run.
Keep the verified production fix (Bash-authoring in run_proposer); the proposer
sandbox/containment stays covered by the existing Bash+MCP and process-tree
canaries.

* test(cli): drop run-analyze.ts from the windowsHide spawn-family list

U7 moved run-analyze.ts's only child_process call (the git status --porcelain
dirty check) into storage/git.ts (already covered by this test, with
windowsHide). run-analyze.ts no longer imports a spawn-family function, so the
windowsHide-regression test's 'must have >=1 spawn call' invariant failed for
it. Remove it from SRC_FILES.

42de243e9a ci(release): sync plugin manifests on every version bump (#2445)
The RC path bumped only gitnexus/package.json, so every v1.6.10-rc tag
through rc.28 shipped the four plugin manifest surfaces frozen at 1.6.9
and failed its own unit suite. The npm version lifecycle script now
runs a fail-closed sync whenever npm version executes, in CI or on a
maintainer's laptop; publish.yml verifies the result and stages the
surfaces into the detached release commit, and the stable path refuses
to publish a tag whose manifests drifted. The sync is textual so a
release commit carries a one-line change per surface instead of
reformatting churn.

Design follows the proposal by @100yenadmin in #2445, moved onto the
standard npm version hook.

RING4-1: all 16 production languages (incl. Vue #940) are registry-primary, so
the legacy resolution legs only ran under the now-removed CI parity gate. Calls
and inheritance now resolve exclusively through scope-resolution
(Registry.lookup, preEmitInheritanceEdges, emitHeritageEdges, buildMro →
MethodDispatchIndex).

Removed:
- Call-resolution DAG: call-processor.ts legacy body (processCalls,
  processCallsFromExtracted, resolveCallTarget + all resolver/dispatch/chain
  helpers), model/resolve.ts MRO-via-HeritageMap, model/heritage-map.ts,
  type-env DAG types; inferImplicitReceiver/selectDispatch LanguageProvider
  hooks + Ruby impls; DispatchDecision/ImplicitReceiverOverride/ReceiverEnriched.
- Legacy heritage path: heritage-processor.ts, heritage-types.ts,
  heritage-extractors/, @heritage.* tree-sitter queries, heritageExtractor/
  heritageDefaultEdge/interfaceNamePattern wiring, worker + parse-impl heritage
  passes (parse-worker/parsing-processor lockstep), cross-file-impl DAG pass.
- Scope-parity infrastructure entirely (no legacy↔registry parity left to run):

PR #1627's npm install -g npm@latest step crashed mid-install with MODULE_NOT_FOUND: promise-retry — a known fragility when npm self-upgrades. Node 22's bundled npm is 10.9.x (no OIDC). Fix: bump publish job's node-version to 24, which ships with npm 11.x natively. Package consumers unaffected (this Node version is only used during publish; engines.node is >=22.0.0; ci-tests.yml continues testing on Node 22).

fix(ci): engage npm Trusted Publishing OIDC properly (#1627)
First live-fire RC publish after #1610 failed at npm publish with E404. The if: failure() cleanup correctly auto-deleted the partial v-tag and rc-marker, but OIDC never engaged. Root cause: two coordinated upstream bugs.

1. actions/setup-node@v6 with registry-url: writes _authToken into the runner .npmrc AND exports NODE_AUTH_TOKEN from its token: input (defaulting to github.token). npm publish sends GITHUB_TOKEN as the bearer and the registry returns 404. OIDC never tried because npm thinks it already has a credential. See actions/setup-node#1440.

2. The Node 22 runner ships with npm 10.9.x. npm Trusted Publishing OIDC support requires npm >= 11.5.1.

Fix: omit registry-url: from the setup-node step (per the consensus workaround in community discussion #176761), and add npm install -g npm@latest before publish. --provenance flag is NOT added; npm auto-attaches provenance under Trusted Publishing.

refactor(ci): unify release pipeline under publish.yml (#1610)
Collapse release-candidate.yml into publish.yml so there is exactly one workflow that publishes gitnexus to npm, creates GitHub Releases, and triggers Docker builds — for both release candidates and stable releases. Closes #1609 architecturally.

A first-stage `route` job classifies push-to-main / push-tag / workflow_dispatch into `rc` / `stable` modes and fails closed on malformed shapes. RC path runs rc-guard → ci.yml → publish (mint GitHub App token → checkout with persist-credentials:false → resolve next rc version → atomic v-tag + rc/<SHA> marker push → vtag integrity gate → npm publish via OIDC → GitHub prerelease → if: failure() cleanup) → docker.yml. Stable path verifies package.json matches the tag and publishes to `latest` via OIDC (no docker).

Hardening:

  • Self-trigger prevention via negative-glob `tags: ['v*', '!v*-rc.*']` — the bug class behind #1609 cannot recur.
  • Two distinct actions/checkout steps per mode (no conditional `token:` expression footgun).
  • Workflow-level `permissions: {}` deny-all + per-job grants; `id-token: write` only where OIDC is used.
  • npm Trusted Publishing replaces NPM_TOKEN (delete the secret after the first successful publish).
  • GitHub App installation token (actions/create-github-app-token@v3.2.0) replaces the long-lived RELEASE_PUSH_TOKEN PAT (delete after first successful RC).
  • vtag integrity gate fails closed on empty / mode-mismatched output (prevents Release named `main` from a github.ref fallback).
  • Annotation-injection sanitization on every logged ref.
  • Explicit `secrets:` passthrough on docker.yml (DOCKERHUB_USERNAME, DOCKERHUB_TOKEN); ci.yml no longer inherits anything.
  • `if: failure()` cleanup auto-deletes v-tag + rc-marker on partial failure (eliminates the external-consumer phantom-version ingestion window).
  • ACTIONS_STEP_DEBUG window closed via `set +x` wrap on the inline auth-header compute.
  • Curated retry-loud error handling on `gh api` bot-user-id lookup and `npx semver`.

Pre-merge validation:

  • 10-reviewer multi-agent code-review pass; 14 findings fixed inline (commit 820cefae), 6 deferred to follow-ups.
  • End-to-end dry-run rehearsal via workflow_dispatch (run 25919563064) validated route classification, rc-guard, App token mint, RC checkout, version resolver, vtag synthetic-regex check, and faithful tarball pack at the bumped version.
  • All zizmor findings on the unification commits closed.
  • Branch-protection required checks all green.

Post-merge actions:

  • After the first successful RC, delete the `NPM_TOKEN` and `RELEASE_PUSH_TOKEN` secrets — they are no longer used.
  • The first real RC after merge is the live-fire test for steps dry-run could not exercise (atomic tag push, real npm OIDC handshake, GitHub Release creation, docker.yml under explicit secrets passthrough). The if: failure() cleanup step handles the partial-failure recovery automatically; the Rollback Runbook in CONTRIBUTING.md covers the rare cases auto-cleanup can't reach.

Scorecard's TokenPermissions check accepts both forms, so this preserves U9 compliance.

* fix(security): narrow permissions: read-all to contents: read on 13 workflows

PR #1454 added permissions: read-all to 13 workflows that previously had no top-level permissions block. read-all is Scorecard-compliant but unnecessarily broad — every job in scope only needs contents:read at the workflow level (job-level blocks already grant the writes that any job actually performs).

Snapshot of every job in the 13 workflows confirms contents:read is sufficient:

- ci.yml: quality/tests/scope-parity have explicit contents:read job blocks; save-pr-meta uses upload-artifact only (no token scopes needed); ci-status is pure shell.
- ci-e2e.yml, ci-quality.yml, ci-scope-parity.yml, ci-tests.yml: all jobs do checkout + npm + tsc/vitest/playwright/upload-artifact only; no API token scopes required.
- claude.yml, codeql.yml, dependency-review.yml, docker.yml, gitleaks.yml, pr-labeler.yml, trivy.yml, workflow-lint.yml: all jobs already declare their own job-level blocks (security-events:write, pull-requests:write, packages:write, etc.) so the workflow-level default does not gate them.

zizmor (--min-severity high) is clean on the resulting tree. Pre-existing medium findings (secrets-inherit, artipacked) are in unrelated workflows and untouched by this commit.

scorecard.yml also uses read-all but pre-existed PR #1454 and is deferred to a follow-up PR per the plan's scope boundary.

recorded is resolved on this branch.

* fix(ci-security): clear remaining zizmor findings

After landing the new security workflows, zizmor reported 5 high+
findings against pre-existing workflows (none introduced by this PR's
new files, all introduced by zizmor's wider scope). Resolved per
research at docs.zizmor.sh and PyO3/maturin issue #2425:

Real fixes (cache-poisoning):
- publish.yml + release-candidate.yml: add 'package-manager-cache:
  false' to actions/setup-node. setup-node v5+ enables caching by
  default when a packageManager field is present in package.json;
  explicit opt-out keeps release installs hermetic and clears the
  audit. Cost: ~30s slower per release run.

Documented exemptions (dangerous-triggers, .github/zizmor.yml):
- ci-report.yml: workflow_run is REQUIRED to post sticky comments
  on fork PRs (forks have read-only GITHUB_TOKEN on pull_request).
- claude.yml: pull_request_target is required by claude-code-action
  to access secrets and post fork-PR review comments. PR checkouts
  pin fork HEAD SHA to mitigate TOCTOU.
- pr-labeler.yml: pull_request_target on the autolabel job needs
  pull-requests:write. release-drafter runs with dry-run:true and
  reads config from the BASE ref only.

Each exemption carries the documented mitigation in zizmor.yml.
workflow-lint.yml now passes --config to both the SARIF and the
gate invocations.

* ci: standardize workflow concurrency and automate release-note labeling

Concurrency — prevent racing CI jobs
 - Every top-level workflow now declares an explicit concurrency block.
 - PR runs cancel-in-progress on supersede; main/push/workflow_call/publish
   runs queue instead of cancelling so every commit and every release is
   validated end-to-end.
 - ci.yml uses a literal `CI-` prefix (not `${{ github.workflow }}`) and a
   per-run nested group for workflow_call invocations, avoiding a potential
   deadlock with publish.yml and release-candidate.yml callers whose own
   concurrency groups could otherwise collide with the called workflow.
 - ci-report.yml falls back to `<head-repo>/<head-branch>` for fork PRs
   (stable across reruns) instead of the per-run-unique workflow_run.id
   which did not actually serialize anything.
 - ci-quality.yml enforces the convention: fails CI if any non-reusable
   workflow lacks a concurrency block or a reusable workflow declares one.

Release-note automation
 - New pr-labeler.yml: amannn/action-semantic-pull-request enforces
   conventional-commit PR titles on pull_request (fork-safe, read-only);
   release-drafter/release-drafter with disable-releaser: true applies the
   matching label under pull_request_target (write-scoped). sync-labels in
   .github/release-drafter.yml removes managed autolabels that no longer
   match (e.g. when `!` or `BREAKING CHANGE:` is dropped from a PR).
 - .github/release.yml (unchanged) continues to map labels to categorized
   release-notes sections.
 - dependabot.yml added for the github-actions ecosystem so pinned SHAs
   auto-refresh on a weekly cadence.

Docs
 - CONTRIBUTING.md documents the concurrency convention, the
   conventional-commit PR-title rules, and the reusable-workflow exception.

Follow-up to verify before relying on the labeler in anger
 - gh api repos/amannn/action-semantic-pull-request/git/refs/tags/v5.5.3
 - gh api repos/release-drafter/release-drafter/git/refs/tags/v6.0.0
 - Confirm release-drafter reads its config from the base ref (not fork
   head) when invoked via pull_request_target.

* ci: address PR review feedback on concurrency and labeler workflows

Two blocking fixes
 - pr-labeler.yml: separate concurrency slots for pull_request and
   pull_request_target. Previously both triggers shared a single group
   with cancel-in-progress: true, so the privileged autolabel run could
   cancel the title-validation check mid-run and leave a required status
   in a permanent cancelled state.
 - pr-labeler.yml autolabel job: add contents: read. release-drafter's
   context.config() reads .github/release-drafter.yml from the default
   branch via the repo-contents API and 403s without the scope. Job-level
   permissions nullify all unlisted scopes so an explicit grant is needed.

Two non-blocking improvements
 - Replace the hardcoded reusable-workflow allowlist in ci-quality.yml
   with dynamic on:-block parsing. New workflow_call-only workflows no
   longer produce false-positive convention failures.
 - Implement actual group-key validation. The check now also asserts that
   every concurrency.group expression references either ${{ github.workflow }}
   or the literal CI- prefix (the documented ci.yml exception).
 - Script extracted to .github/scripts/check-workflow-concurrency.py so
   it is runnable locally and independently testable.

fix: bundle gitnexus-shared into CLI dist (#613)
* fix(ci): build gitnexus-shared before publish, use CHANGELOG for release notes

The publish workflow was missing the gitnexus-shared build step that
the setup-gitnexus composite action provides. Since PR #536 unified
the ingestion pipeline, gitnexus imports types from gitnexus-shared,
so it must be built first.

Also replaces generate_release_notes with CHANGELOG.md extraction so
GitHub Releases use the reviewed changelog entry instead of a flat
PR title list.

* fix: bundle gitnexus-shared into CLI dist to fix module resolution

gitnexus-shared was declared as a file: dependency but never published
to npm, causing ERR_MODULE_NOT_FOUND for users installing gitnexus
globally. The build script now copies gitnexus-shared/dist into
dist/_shared/ and rewrites bare specifiers to relative paths.
