fix: add dedicated COPY query for A11ySignal (column mismatch)

A11ySignal was falling through to the generic 7-column default in
getCopyQuery(), but the DDL and CSV both define 10 columns. This caused
a column mismatch on COPY load — the same class of bug as PR #559.
