fix: register state-slots phase in pipeline + index

Browse source 
Wire stateSlotsPhase into the barrel export (pipeline-phases/index.ts)
and into buildPhaseList in pipeline.ts (after crossFilePhase, matching
declared @deps). Phase was implemented but never executed — StateSlot
nodes and data_flow edges were silently dropped.
