fix(resolution): label name-fallback guesses and refuse the impossible; bind exports and named imports to module-level declarations

The global unique-name call fallback emitted its edges as 'import-resolved' at
confidence 0.85 — byte-identical to a genuinely resolved edge, so nothing
downstream could tell a guess from a fact. Those edges now carry reason
'global-name-fallback' at 0.5; per-language visibility hooks refuse the edges a
language makes impossible (Go unexported names across packages, non-pub Rust
items, Swift private/fileprivate, Dart _private across libraries, Ruby class-owned
methods with no namespace evidence); Dart and Swift stay labeled where the parse
model cannot decide.

In the shared finalize pass, 'export *' fan-out and (for TS/JS/Vue, via a
ScopeResolver hook) named imports bind module-level declarations only — a class
method can no longer win a barrel name — and a star-vs-star collision is refused
and recorded as 'reexport-ambiguous' instead of first-wins. The reason now rides
on forEachRelationshipFields so consumers can gate on it.

Each run logs a census (guessed sites, distinct pairs, refused, ambiguous names)
and persists it as RepoMeta.nameFallbackEdges.
